I acknowledge that it is my responsibility to disclose all allergies, sensitivities, medical conditions, pregnancy, injuries, scalp or skin conditions, or any condition that may prevent me from safely lying on a treatment bed prior to receiving services. I understand that some products may contain nut-derived oils, essential oils, or botanical ingredients. The Zen Room is not responsible for reactions or complications resulting from undisclosed conditions or allergies. I release The Zen Room and its provider(s) from liability related to information I failed to disclose. Services may be modified or refused at the provider's discretion for safety reasons. • I have disclosed all allergies, sensitivities, and medical conditions • I understand essential oils and topical products may be used unless I request unscented products Cancellation, Deposit & Redo Policy A $50 deposit is required to book all services and will be applied to the appointment total. Cancellations or reschedules must be made at least 24 hours in advance or the deposit will be forfeited. Clients arriving more than 15 minutes late may have their service time reduced or the appointment cancelled at the provider's discretion.
